Life Skills Support Worker - Northern Health

Hours: Permanent part time (0.40 FTE)
Wage: $21.34 - $23.15 per hour.
Qualifications:  Diploma from a recognized Social Services Support Worker educational program, or Residential
Care Aide program, two (2) years' recent, related experience working with people with complex health issues, or an equivalent combination of education, training and experience.
Or Grade 12 plus a Community Mental Health Worker Certificate and Addictions Studies Certificate plus three (3) years recent related experience working with adults with mental illness and/or addictions diagnosis; or an equivalent combination of education, training and experience.
Valid BC driver's license.
Valid First Aid Certificate, CPR, and Food Safe.
Job Duties:  In accordance with Northern Health's vision of an idealized system of services built on a foundation of
Primary Care where people and their families receive primary care services in Primary Care Homes supported by inter-professional teams the Primary Care Life Skills Support Worker functions as a member of the inter-professional team, under the direction of the Team Leader.  This position applies best practices to provide people with services as outlined in an individual's plan of care, with the goal of maximizing independent living.
